Reference Cards are made up of five fields, labelled Author, Title, Pub., Notes and Key. "Pub." is intended to comprise all publication information, including publisher, place of publication, journal, volume no., etc. If "Title" refers to an article included in an edited volume, the book title and editor's name would be included under "Pub." Each field should include the relevant punctuation so that the first three fields can be concatenated to form a standard bibliographic entry (see the 3 samples following the present card.)
"Notes" is a miscellaneous field. I often use it to note the location of my sources (e.g., photocopy, library catalogue number). "Key" is for keywords suitable for sorting your database.
